  The prominence of the chin & nose & cheek bones make them vulnerable to fractures in accidental injuries. As in any disease proper treatment by proper person depends on the success of the treatment. Each bone has a specific function & the mobile bone a specific joint. The super specialist understands this very well. As he is the person who knows more and more about less & less! The function of the jaw bones is to maintain the esthetics of the patient. They are also extremely useful in chewing & speaking.

In accidental injuries when the doctor has not seen the patient before accident he cannot judge the face of the patient. The only thing that is constant on the face is the relation ship of the upper & the lower teeth while chewing. If we can get the teeth in proper position the face will appear normal!

The upper jaw is called maxilla, the lower jaw is called the mandible. The lower jaw is mobile the upper jaw is not. The upper jaw that is the maxilla is fused with the many of the other facial bones like…zygoma (the cheek bone), nasal bones & vomer to name a few. In accidental injuries all these bones do not fracture individually!! They fracture on the weak lines researched by Le Fort. The treatment of all these fracture depends on…the occlusion of upper & lower teeth, & the perfect understanding of the temporomandibular joint. The knowledge of dental sciences is must for the proper treatment of these fractures.

The surgery should be done from the mouth so that there is no scar, and many times it can very conveniently done under local anaesthesia.

 The upper & the lower jaws have a very specific relationship. some times the growth of one jaw may be more then the other. this can cause facial asymmetry. this will affect the facial apprearance. this needs surgery which can change the personallity of the person. this will affect the speech, looks & ability to chew food. the person can be confident in the society & in profession
 

Impacted wisdom molar

 This means the third molar. Humans have three molars the first second & the third. All these teeth are four in number. One in the upper right quadrant of the mouth second in the upper left quadrant & similarly lower left & lower right quadrants. They erupt in the mouth between the age of 18 to 25 years. This is the time when a person is supposed to get mature or become wise.

The reasons that this third molars do not erupt normally in most persons are… during evolution of mankind, our food has become soft, more cooked so we may get away without one pair of molars; in order to chew food. Secondly the jaw size has reduced. The third molars erupt last & at this time the jaw bone growth is almost complete. This might be the reason that there is not enough space for them. There are many theories for finding the right cause of these third molars being impacted against the other tooth or the bone or the mucosa (the skin of the mouth).

In fact, we should be interested in the solution of the problem rather then wasting time after the cause. THE ONLY SOLUTION IS REMOVAL OF THE THIRD MOLARS.

IMPACTED, EMBEDDED, UN ERUPTED…such words are used for the teeth which do not properly position themselves in the mouth. They may be a bit tilted in any direction. This can cause many problems to name a few…they may damage to the adjacent teeth, some structures in the bone, cause infection in the overlying mucosa, the skin around the crown may differentiate into some sort of tumour or a cystic lesion and so on…

The surgical procedure to remove such teeth is a bit tricky. Basic fundamentals of surgery & gentle approach make the recovery simple. This is a very delicate surgery & has to be performed meticulously & with extreme care.
Strict antiseptic condition should be maitained.

 All the research in the medical & dental science is based on offering better & permanent treatment of any disease. There are a few inventions in medical science that has drastically changed the mode & out come of the treatment given; especially in dental science; one is the X-rays & the other is the local anaesthesia. The third one which fascinates me the most is THE DENTAL IMPLANT! This modality of treatment replaces tooth in place of a tooth. It replaces the root & the crown; without the possibility of caries! This is the major difference between the tooth & the implant; the tooth can decay the implant can not! So no root canal therapy for implants!

Each dental surgeon wants to save the diseased tooth. Root canal treatment is one of such fascinating treatment modality. The blood vessels & the nerve (pulp) in the centre of the tooth dies. The cause may be caries, trauma or any thing that can harm the pulp. Even with the most sincere attempts by the dental surgeon the infection may not completely be cured. The bone above the tip(apex) of the root of the tooth may remain infected. It may cause some granuloma or a cyst. Worst it can affect the tip of the adjacent tooth. This may go on spreading.






The surgical removal or curettage of this infected bone & the removal of the some part of the tip of the root is called apicectomy. In short, the infection in the root canal of the tooth is cleared & treated with root canal therapy & the infection in the bone & around the apex of the root is cleared by apicectomy. Utmost care must be taken to remove all the infected tissue around the root tip; especially the palatal or the lingual aspect.
This may be the last attempt to save the affected tooth.
